case management
Our team provides much needed case-management as an additional service to all of our clients. This includes general assistance filling out applications for Medicaid, Housing applications, and numerous other assistance programs. We are tasked with helping people we encounter with any service they may need. Through dialogue and case management, we are better able to adminster referrals, track, follow up with, and assist the individuals we work with.

Daily outreach/ mobile partnership
The organization moved into a new space in 2019 to be closer to the individuals that we serve. With the covid-19 outbreak we made some changes due to CDC guidelines and created a mobile outreach partnership with Aurora Mental Health and The Salvation Army stopping at numerous locations along East Colfax.
our mobile outreach team provides case management, general hygiene items, and non-perishable food kits. This new partnership has vastly expanded our reach to individuals, and we are now experiencing a 50 percent increase in weekly connections.
